How to Apply for ACP

If you are a survivor of certain crimes, or you are a legally-protected health care provider, the Address Confidentiality Program (ACP) can help protect your address from public disclosure.

Choose an option below to learn more about the program and how to apply to become an ACP participant.

Survivors

Select this option if you are a survivor of:

  • Sexual assault
  • Rape
  • Stalking
