How to Apply
Step 1: Review the application instructions
The MHC strongly encourages all potential applicants to view the following presentations, familiarize themselves with the application requirements, and begin compiling application materials as early as possible to meet the application deadline.

Step 2: Download and save the application form
Download and save a copy of the MPPF Application. If you need a hard copy of the Application and instructions by mail, contact the MHC office.

Step 3: Complete and print the application form
Enter your information and print the application. The file contains rights for fill-in and save capabilities, but only when using Adobe Reader.
Completed applications must include:
- A printed copy of the application form and all required attachments in a simple three ring binder. All required signatures must be live and in blue pen.
- A USB flash drive containing digital photographs requested in Section I and Section IV (each elevation of the resource and details of proposed project areas)
Step 4: Mail your application to the MHC
Mail your completed application to the MHC to:
