Filing by Fax

Fax Filings are available for UCC and Business Entity Filings. All filings submitted by fax must have a bar coded Fax Voucher Coversheet as the only coversheet. Any fax filing submitted without the bar coded Fax Voucher Coversheet will be rejected.

PLEASE NOTE: A Fax Voucher Coversheet may only be used once.


Three Easy Steps to File by Fax

#1 - Locate and Complete your Filing Form

To locate a filing form:

  1. Go to the Mail or Walk-in page
  2. Locate your section
  3. Select the type of company
  4. Select the form you need

#2 - Create the Fax Voucher Coversheet

Click on the Create Fax Voucher Coversheet link below. Complete the form. Select your filing category and filing type. You may need to answer additional questions. The system will automatically calculate the correct filing fee. When the form is complete, you will be taken to a secure payment page. You can be with Visa, MasterCard, American Express, a debit account, or an e-check.

You’ll have your fax cover sheet and payment confirmation. This will be used as your fax filing.

#3 - Assemble and send your fax

Place the Fax Voucher Coversheet, as the first and only coversheet, on top of your fax filing and fax to the number listed on the Fax Voucher Coversheet.

Your fax filing will generally be processed the same business day but in no event later than by the following business day. You may confirm your filing by searching for it on the Corporate Database. If you do not see your filing it may not have been processed or has been rejected. The see if it has been rejected, search the Corporations Rejected Filings Viewer.

In all cases where you have difficulty, please call the Corporations Help Desk at 617-727-9640 between the hours of 8:45 a.m. and 5:00 p.m. Monday through Friday. between the hours of 8:45 a.m. and 5:00 p.m. Monday through Friday. Please have a copy of your fax when you call. If you had difficulty printing the Fax Voucher Coversheet after you have completed payment, please have the Fax Voucher Number available for the Help Desk. The Fax Voucher Number can be found on your Fax Voucher Coversheet/Payment Confirmation.

How does it work?

Our system automatically sorts filings that are faxed to our office. The system accomplishes this by reading a barcode at the top of the Fax Voucher Coversheet. Special instructions encoded in the barcodes instruct the system how to route the filing correctly for approval, data entry and other functions. Special fax servers handle the receipt of all fax filings, so there's no need for standard fax machines and paper. It's electronic and less time consuming.

What do I need to do?

To successfully submit a fax filing, you must use a Fax Voucher Coversheet specifically designed for your type of filing. This combination of a Fax Coversheet and Payment Voucher not only assures your filing is properly directed but also provides a secure payment process. The use of an incorrect Fax Voucher Coversheet or an incorrect payment amount will result in the filing being misdirected by the automated system and ultimately rejected. You must use the special Fax Voucher Coversheet as the only coversheet. Please do not add your own coversheet. How do I know if it arrived at the SOC?

Most fax machines produce reports indicating that a fax has been sent successfully. Please reference your fax machine or fax software documentation. The typical turn around for a fax filing submitted during business hours is the same business day. Filings submitted on weekends or after hours will not be processed until the next business day. Please allow at least 48 hours and then check by searching the corporate database, UCC database, or trademark database to see if your filing has been added to the system. If for any reason you are not able to view a filing, please contact our Corporations Help Desk at 617-727-9640. Failure to view a filing may mean that it was rejected. If your business entity filing was rejected, a copy of the rejection sheet and rejected document will be available here. If a fax filing was rejected, you will be notified by electronic mail or the postage service.
