Voter Registration Form Request

To request a voter registration form to register to vote in Massachusetts, please completely fill out the information below and submit. A form will be mailed to you on the next business day following your request.

You will not be registered to vote until you return the completed voter registration form to your local city/town hall.

You can also download a Massachusetts Mail In Voter Registration Form which will allow you more time to send your completed form to your local election official.

To register to vote in other states, you can download a National Voter Registration form.
