• credit card required for payment
• in "Special filing instructions" box, type SEND TO Worcester North
• order by corporation's Federal ID number

Instructions for online orders

1. the link below takes you to "Search Corporate Database"
2. enter the name of the corporation
3. select the correct entry from the resulting names
4. copy that corporation's Federal ID number
5. click "Request a Certificate" and complete that form
6. in "Special Instructions" type SEND TO Worcester North
